Skills Inventory and Consultation

An individual’s goals and preferences mold a plan for success. Through a person-family centered planning process, consultation guides families as they explore options and opportunities, assess “best fit” living and job opportunities, and develop individualized home programs to foster independence and other readiness training. What is your family’s vision for the future? How do you engage in effective training and self-determination practices at home? What community opportunities and resources are available to build these “next steps?” More specifically, what job setting do you prefer? What might be the ideal living arrangement and what tools/supports can help to create this? Are there post-secondary programs or other supports available to assist with this transition? Consulting is available to help explore this process with you.

Resources related to funding opportunities and other transition experiences are available. Consultation may consist of phone/email communication, home visits, and “on-the-site” training. Using the Full Life Process™ model of Center for Independent Futures, we support families through the readiness process.

Personal Support Networks

Carousel Community Connectors assist families to establish goals and aspirations, share with network members, facilitate gatherings, and develop and implement a work plan.

A “circle of care” or Personal Support Network consists of members who form a relationship with the person at the “center” and other members of the network. The network ensures that an individual experiences a joyful, meaningful, safe, and secure life through advocacy and companionship.
